Form	Description
audition information	Please print the Audition Information form, complete and sign, and bring to the auditions.
emergency information	If cast in the production, a completed and signed Emergency Information form is required prior to participating in rehearsals.
parental consent	A completed and signed Parental Consent form is required to audition.
technical registration	Students preferring Technical Theatre, please print the Technical Registration form, complete and sign, and bring to the auditions.
waiver	If cast in the production, a completed and signed Waiver of Liability form is required prior to participating in rehearsals.
